Incident Reporting

The system is confidential

You can use the incident reporting form to report an incident, near miss or safety concern that occurred during any paddling or surfing activity.

Reporting an incident helps us to understand the types of incidents occurring and identify trends. This helps to inform our guidance and enables us to address safety issues and concerns.

Our approach to incident reporting is based on learning from what went wrong and preventing similar incidents from happening in the future. It is not designed to point blame or result in negative consequences.

As a Club we take safety seriously, incidents the first instance should be collated on the clubs reporting form. In addition to this if we are at a centre such as Lee Valley we would also enter the incident on thier reporting system. 


We also need to place an incident on to theExternal link opens in new tab or windowPaddle UK Incident reporting portal The format of the PUK portal is similar to the Club which means you only need to input the details once and the details will be easy to copy over to the PUK site. This will help stream line the process of transfering the information.
